Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. Charges are payable at the time of treatment. Our office accepts all major credit cards except American Express. Outside financing is available through CareCredit upon approval.
Please ask a member of our staff, and we will review our payment options to accommodate your financial needs.
If special arrangements are needed, please talk to our office manager prior to receiving service.
Additionally, we will submit all dental insurance claims for you. We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your policy has not paid within 30 days. We are a premier provider for Delta Dental.
At this time we do not accept Medicare or Medicaid but do offer great payment plans and self-pay rates.
Temporary Change Due to COVID-19
Dr. Evanson and our team’s mission is to provide you the highest care possible, and your health and safety are of our utmost concern. As a result, while we transition to being able to service both emergency and elective procedures, we have implemented additional measures which include using an increase in Personal Protection Equipment (PPE) in accordance with the ADA guidelines for COVID-19 infection control protocols. We’ve worked hard to minimize any additional cost to our patients where possible, but due to the necessary changes, hygiene and restorative appointments will temporarily be increased by $10 to help cover the cost of the required PPE. Please bear in mind some dental insurance providers will cover this additional charge, and as a courtesy, we are willing to submit this on your behalf (although the patient is ultimately responsible for this small charge should insurance not cover the cost).
